I agree that my child will attend and I will assume all of the risks of any art activities, whether known or unknown and do hereby release and forever discharge Mary Magdalena, her employee, volunteers and agents of any liability with respect to any injury, illness, damage or death occurring to my child while he/she participates in any activities. The students will be using glue guns unless you request that they can’t. We will be painting with acrylics, so have them wear appropriate clothing, it doesn’t wash out. Photographs of the children and their work can be used for advertising purposes.

My child is able to participate in all art activities and has my permission to do so. In case of emergency, I understand that every effort will be made to contact me. In the event that I cannot be reached by phone I hereby give permission to the physician selected by the camp director to secure proper treatment for my child and that this constitutes a release for the treating physician and guarantees payment.
